Please note that the information provided in this survey may be verified via an on-site visit by a representative of U.S. Customs and Border Protection (CBP) and/or the U.S. firm that requested the completion of this survey. The on-site visit will consist of an audit that will include a physical inspection of the facility and a review of the corresponding security processes and procedures consistent with the CTPAT minimum security criteria. This applies to all business partners, regardless of whether they are CTPAT members or non-CTPAT members, and regardless of whether they are eligible or not eligible to join the CTPAT program.
Please answer the questions based upon what processes and procedures your facility currently has in place. The purpose of this survey is to determine if your facility is meeting CBP’s minimum security criteria for CTPAT. If your facility is not meeting the CTPAT minimum security criteria, we can assist you to meet the criteria. However, if during the on-site visit, CBP determines that your facility is not meeting the CTPAT minimum security criteria, then the U.S. company that has requested you to complete this survey may be suspended from the CTPAT program.

Directions for Completion of the CTPAT Security Survey
Once you have completed the CTPAT security survey form, please save it and e-mail it back to the company that directed you to this web page at your earliest convenience. Please complete everything on the PDF and attach the two signature pages as a separate document. If this is not possible, you can print it and fax (or mail) it. Simply completing the form online does not “submit” it to us or to our consulting firm.
